This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
1. Get an Electronic or Paper Copy of Your Medical Record
- You have the right to see or receive an electronic or paper copy of your medical record and other health information we have about you.
- Ask us how to request this information.
- We will provide a copy or summary usually within 30 days of your request.
- A reasonable, cost-based fee may apply.
2. Ask Us to Correct Your Medical Record
- You can request a correction to any health information you believe is incorrect or incomplete.
- Ask us how to submit a correction request.
- If we deny your request, we will explain the reason in writing within 60 days.
3. Request Confidential Communications
- You can ask us to contact you in a specific way (e.g., by home phone or office phone) or to send mail to a different address.
- We will agree to all reasonable requests.
4. Ask Us to Limit What We Use or Share
- You may request that we do not use or share certain health information for treatment, payment, or health care operations.
- We are not required to agree to your request and may deny it if it could affect your care.
- If you pay in full out-of-pocket for a health care item or service, you can ask us not to share that information with your health insurer for payment or operations.
- We will agree to this request unless a law requires us to share the information.
5. Get a List of Those With Whom We’ve Shared Information
- You can request an accounting of the times we’ve shared your health information in the six years prior to your request.
- This list will include who we shared it with and why, except for disclosures related to treatment, payment, health care operations, or those made at your request.
- The first request each year is free. We may charge a reasonable, cost-based fee for additional requests within the same 12-month period.
6. Get a Copy of This Privacy Notice
- You can request a paper copy of this notice at any time, even if you previously agreed to receive it electronically.
- We will promptly provide you with a printed copy.
7. Choose Someone to Act for You
- If you have appointed someone as your medical power of attorney or legal guardian, that person may exercise your rights and make decisions about your health information.
- We will verify the individual’s authority before acting on their request.
8. File a Complaint if You Feel Your Rights Are Violated
- You may file a complaint with us if you believe your rights have been violated. Contact information can be found on page 1 of this notice.

- We will not retaliate against you for filing a complaint.
When You Have the Right and Choice to Tell Us “Yes” or “No”
You can tell us to:
- Share information with your family, close friends, or others involved in your care
- Share information in a disaster relief situation
- Include your information in a hospital directory
- Contact you for fundraising efforts
If you are unable to tell us your preference—for example, if you are unconscious—we may still share information if we believe it is in your best interest. We may also share information to help address a serious and imminent threat to health or safety.
Situations Where We Will Never Share Your Information Without Written Permission
We will not share your information for the following purposes unless you provide written authorization:

- Sale of your information
- Most sharing of psychotherapy notes
Fundraising Communications

- You have the right to ask us not to contact you again, and we will honor your request.
How We Typically Use or Share Your Health Information
We may use or share your health information in the following ways to support your care and ensure you receive the services you need.
1. Treat You
We can use your health information and share it with other professionals who are treating you.
Example: A doctor treating you for an injury may ask another doctor about your overall health condition.
2. Run Our Organization
We can use and share your health information to operate our practice, improve your care, and contact you when necessary.
Example: We use health information about you to manage your treatment and services.
3. Bill for Your Services
We can use and share your health information to bill and receive payment from health plans or other entities.

Changes to the Terms of This Notice
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our web site. This notice is effective dated June 2nd, 2025.

Who we share your data with
If you request a password reset, your IP address will be included in the reset email.
How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
